Huh, that's interesting. So doctor is Doktor when used as a title, but Arzt as a general noun?

Doktor is a title. You do not need to have a medical degree to be a doctor, eg a doctorate in engineering or philosophy.
Arzt is a medical doctor, although many medical doctors are also Doktors.

People in Germany often are calling every doctor in Germany Doktor.
But I would say it is more a colloquialism. Their profession is Arzt and the academic title is Doktor. I guess it is becauce most Ärzte have also a doctor title. In a more formal speech we wouldn't use Doktor for the profession.
