Cosa fa is the polite Form. Fa' also exists, it's the imperative: Fa' (also fai) qualcosa. The polite form of the imperative is faccia.

My understanding is that Cosa fai? is equivalent to “What are you doing?” or “What do you do?”, while Cosa fa’? is more or less equivalent to “Whatcha doin’?”

Presente Present tense
io faccio - I do
tu fai - you do
egli/ella fa - he/she does
noi facciamo - we do
voi fate - you do
essi fanno - they do

Imperativo presente
(tu) fai / fa’
(egli/ella) faccia
(noi) facciamo
(voi) fate
(essi). facciano

Watcha doin' would rather be
"Ma che stai facendo?" There is a form corresponding to the progressive form in Italian, it's built with the forms of "stare" (sto, stai, sta, stiamo, state, stanno + gerund (ending in -ando o - endo).
