In general:

איזה - which

מה - what

When "what", in English, can be replaced with "which", you use איזה (or איזו for feminine nouns).

Could someone elaborate more about the differences?

מה is a question about the subject or object of the sentence. (unless there's a preposition before it, then it can also be other things.)
איזה is a question about a modifier. It must always be followed by the noun which this modifier describes.
If "what" actually means "what kind of", use איזה.
