A nice write-up. I'd add one more item: if all of the above still does not work as expected do not be too hard on yourself. Take a step back, relax, take a break from learning for a while. That might help. Language learning may be very ungrateful: no matter how hard you try, when you start learning as an adult you will never become as good as a native (with few exceptions), you will have an accent, make grammar mistakes use wrong words. But that is not a reason to give up.
